At Crann we are pioneering a new model of care for disability. We are the first organisation in Europe to adapt the Aspen Institute’s 2 Generation (2Gen) approach, meaning we support both you and your family throughout all stages of life.

Our Model of Care is focused on 6 services that provides holistic & comprehensive wraparound care programmes with personalised plans for you and your family members.

We will work together to establish goals for you and your family, and create a tailored programme across our range of services that will help you achieve them. As your requirements change, our team will work with you to provide life-long support.
