Job Title: Health and Continence Nurse

Location: The Crann Centre, Ovens/Ballincollig, Cork 

Reporting to: Health and Continence Manager

Part-time (22.5 HRs per week)

Role Profile

This is an excellent opportunity for an experienced, motivated Nurse to work as part of a dedicated team in developing health and continence support programmes to increase our client’s abilities to carry out daily activities or occupations independently and with more confidence. 

 

Key responsibilities

  • Support the Health and Continence Manager in developing and growing the continence clinic service
  • Assessing and identifying the individual needs and goals of clients who have a neuro-physical disability to maximise their level of independence in terms of continence management
  • Work with the health and continence manager in identifying health care needs which may need to be addressed
  • Delivering health and continence care programmes and/or interventions to support both adults and children with neuro–physical disabilities to achieve individual goals
  • Deliver training and workshops to families, other HCP, SNA’s, school staff, employers on topics relative to individual client’s needs
  • Liaising with other HCP involved in a person’s care to ensure continuity
  • Work as part of the Crann Client Services MDT
  • Write up reports and session notes in a professional and timely manner
  • Report to the health and continence manager
  • Contribute to data collection, analysis, and reporting on service outcomes.
  • Carry out any other reasonable duties within the overall purpose of the post.

Essential Knowledge, Skills & Experience

  • Experience working with children and adults with disabilities
  • Experience working in the area of continence, preferably neurogenic although not essential as training will be provided
  • Experience with the management of constipation
  • Urological experience where possible esp bladder management (neurogenic experience preferable), ISC/CIC, knowledge of urological investigations
  • Experience of toilet training for children with disabilities
  • Ability to coordinate workload, coordinate clinics and review appointments: Signpost clients to other members of the team where indicated
  • Work as part of a team
  • Able to work on own initiative, within scope of practice.
  • Refer concerns and escalate as needed.
  • Liaise with Consultants/MDT of regional and tertiary referral centres as needed to liaise with client care/referrals/feedback to referrers
  • Positive, practical can-do, solution focused approach
  • Flexible and able to handle change in response to individual and organisational needs
  • Excellent task management, IT and organisational skills
  • Co-facilitate/Facilitate on SNA programmes, inhouse and online education programmes for clients of Crann (children, teens, adults, parents)
  • Awareness of child protection and adult safeguarding
  • Policy development

Qualifications:

  • A recognised Nursing qualification in one or both of the following – Paediatrics and/or General with at least 3 years post qualification experience
  • Candidate must be registered with NMBI
