The Next Step: Transition towards independence (Teens)
This informal talk provides a starting point to help provide the foundations needed to encourage teens and also their parents to start preparations for moving forward.
This can be an exciting time but for some it can be challenging and come with a degree of uncertainty as to what the future will look like. Ideally discussions around transitioning towards adult healthcare services and adulthood in general should ideally commence when teenagers are around 12-14 years old.
This allows time for the teen to learn how to develop the skills and strategies needed to engage more independently, to make decisions and to develop life skills that will help prepare for their transition towards adulthood.
