Crann’s 2Gen Model of Care intentionally puts family at the centre and has 6 interconnected areas of support addressing health, wellbeing, and participation across the lifespan. The 6 services are Psychological Wellbeing, Health & Continence, Education & Career Pathways, Mobility, Independent Living & Social Capital.

The model allows Crann’s team of professionals including nurses, occupational therapists, counsellors, dieticians, reflexologists, podiatrists, education & career mentors and play therapists to deliver comprehensive wraparound care programmes enabling better health & wellbeing outcomes and increased opportunities for participation in education, work and leisure for the children, adults and families who avail of services at Crann.

Our Services

Psychological Wellbeing

Our Psychological Wellbeing service is designed to support adults, children and families in building resilience and sustaining positive mental health. This is a key support for dealing with challenges like trauma or health issues which arise for the person with a disability, their carers, or their family.

In 2023 Crann’s Psychological Wellbeing team delivered 750 client sessions.

Health & Continence

Our comprehensive Health & Continence service offers customised solutions to deal with core issues arising from neuro-physical conditions transforming client & families lives in practical ways so they can engage in daily activities with confidence.

In 2023, Crann’s Health & Continence team delivered over 1,000 client sessions.

Crann also provides a range of training courses and workshops for healthcare professionals and is the only designated trainer for Trans-Anal Irrigation and Antegrade Colonic Enema training for public health nurses in Cork & Kerry. Our continence team also offers Munsters only Clean Intermittent Catheterisation (CIC) training programme for Special Needs Assistants and certified CIC training for nurses.

Education & Career Pathways

Our Education Pathways support children, young adults and their parents to navigate through education at key transitioning stages. Our one-to-one coaching provides the necessary support to guide them through appropriate learning options, accessibility, grant availability, job readiness and further education and career opportunities.

Career Pathways address physical & vocational challenges a person will encounter in the workplace environment with personalised occupational therapy services. Our team provides assessment and direct personalised intervention in the areas of assistive technology (AT), fine motor skills, handwriting and visual perception.

Crann’s Education & Career Pathways team delivered 161 sessions in 2023.

Mobility

Our Mobility service was developed to support independence and accessibility to improve daily life for people living with neuro-physical disability. Programmes include Skills on Wheels which offers one of the few structured wheelchair training programmes in Ireland, as well as programmes including learning to transfer and upper limb rehab.

In 2024 the development and expansion of a dedicated Pediatric OT programme at Crann allowed children and teenage clients with neuro-physical disabilities to receive a comprehensive OT service to meet identified and currently unmet needs for this client group.

Our Occupational Therapy team delivered 195 client sessions for our Mobility services in 2023.

Independent Living

Our experienced team support children, teenagers, and adults to gain greater independence both inside and outside of the home, through a range of therapeutic interventions that focus on cognitive, visual perceptual and functional task training. Our on-site AT clinic offers assessment and personalised programmes.

We support parents with the practical and emotional aspects of preparing and facilitating their teenager or young adult to live independently. In 2024, we added an Independent Living training space for practical skill development & practice.

In 2023, Crann’s Independent Living team delivered 190 client sessions.

Social Capital

Social Capital offers a range of programmes & facilities to alleviate isolation & loneliness, increase peer support & social participation, and improve mental health & wellbeing. This helps children, adults and families to engage with each other and build a network at Crann and in their surrounding communities. Our programmes include Breaking Barriers Theatre Group, Chat & Connect Social Groups, Youth Clubs and Parent’s Café.

In 2022, we opened Ireland’s first fully accessible playground and leisure area which allows our families experience play without barriers and Conservation at Crann which is an accessible conservation area and nature trail.

In 2023 our Social Capital team delivered 1,966 client sessions.

Clients must give consent to be referred to and for information to be passed on to Crann as information will be saved on Crann data bases.
